See the Difference with ICUBA Vision Coverage 👓

ICUBA partners with EyeMed to offer two voluntary vision plans, both featuring a $5 copay for in-network eye exams and coverage for lenses or contacts every 12 months.

What is the difference between the two plans? Frame benefits and allowance!

  • Base Plan: Frames every 24 months with a $130 allowance

  • Buy-Up Plan: Frames every 12 months with a $160 allowance

📲 Our network is in the Insight network! You can find in-network providers through the ICUBA Benefits Portal or EyeMed Website.

Enjoy instant benefits at checkout with free shipping and no paperwork at participating retailers like: LensCrafters, Target Optical, ContactsDirect, Ray-Ban, and Glasses.com.

At participating in-network providers, EyeMed Vision members also receive 💸:

  • 40% off an extra pair of glasses

  • 20% off lenses or frames (when purchased separately)

  • 20% off non-prescription sunglasses and accessories

  • Discounts on LASIK (call 800-988-4221 to locate a provider)

🩺 Remember: medical conditions of the eyes (like glaucoma or cataracts) are covered under your medical insurance from BCBS, not your vision plan! You can view our post about searching for in-network medical providers here.

No card? No problem.

Providers can verify coverage using your Social Security Number. To print a new card or access your digital ID, visit EyeMed.com or log in to the ICUBA Benefits Portal.
