To find in-network providers for ICUBA's medical and behavioral health plans, follow the steps below:

You can search for in-network providers easiest through My Health Toolkit at https://member.myhealthtoolkitfl.com/ or the Embold Health provider guide at https://icuba.emboldhealth.com/login.

Log in: Use your member credentials for My Health Toolkit.
Search for Providers: Utilize the provider search tool to find in-network doctors, specialists, and facilities. Alternatively, you can call Florida Blue Customer Service at (855) 258-9029 for assistance.
You can also use the ICUBA Benefits Portal at http://icubabenefits.org and access your BlueCross BlueShield account under the "My Carrier Accounts" section of the landing page. Once you are in My Health Toolkit, click on the "Find a Doctor" link and follow the prompts to enter your 3-digit alpha prefix (for ICUBA members this is ICI). Click continue and search for a provider.
Coverage doesn't stop at Florida! The BCBS medical and behavioral health provider network is available nationwide. There's even coverage available if you are traveling abroad. As an eligible Blue Cross and Blue Shield member, you can use this website to find doctors and hospitals outside of the United States, Puerto Rico and U.S. Virgin Islands.
