ICUBA partners with Delta Dental to offer three dental plan options designed to meet your needs and budget:
The DHMO (Dental HMO): Offers fixed copays, no deductibles, and no annual maximums through the Dental Care USA Network. It’s a great low-cost option but is not available in all states - check eligibility before enrolling.
The Base PPO: $1,500 annual max, deductibles, and coinsurance for services. Orthodontia is covered for children up to age 18, up to $1,500.
The Buy-Up PPO: $2,000 annual max with broader coverage. Orthodontia is covered for both adults and children, up to $2,000.
