What is Personify Health?

Personify Health is ICUBA’s Wellbeing Incentive program that rewards members (and spouses) enrolled in an ICUBA medical insurance plan. You can earn incentives redeemable for gift cards at many popular retailers after reaching program milestones.

How do I use Personify Health?

Accessing Personify for the first time is easiest through the My Health Toolkit single sign-on with BCBS. After logging in:

• Click on the Wellness & Care Management tab then select "Wellness Programs"

  

• Select Personify Health

• 

• Next click on the Personify link for single sign-on to your Personify account

• 

• Under "More" select "My Profile" then complete your profile and create a username and password to you use when logging in to the Personify Health website or mobile app on Google Play or the App Store.
