Enrolling and Making Changes to Coverage

Know When You Can Enroll or Make Changes to Your Coverage 🩺


You have two main opportunities to enroll or make changes to their health benefits:

  • When you’re first hired, this is your initial enrollment period

  • Each February, during Annual Enrollment for the upcoming plan year (April 1)


But life happens - and thanks to HIPAA, certain Qualifying Life Events (QLEs) allow you to make benefit changes outside of these windows. These events include marriage, divorce, birth or adoption, loss of other coverage, court orders, or becoming eligible for Medicare, Medicaid, or CHIP. In order to make changes for these life events, you have to start a life event with your employer and submit documentation within 30 days of the event (or 60 days for changes in Medicare, Medicaid, or CHIP eligibility).


Most schools allow employees to submit life event requests through the ICUBA Benefits Portal, but some schools use Workday for benefits enrollment and navigation. If you are unsure about where to start your life event, contact your Human Resources or Benefits Department directly.


Please note: Barry University, The University of Tampa, and Florida Tech employees do not use the ICUBA portal for enrollment. Changes for employees at these schools must be made directly in Workday.


Who You Can Cover 👨‍👩‍👧‍👦

When adding dependent(s) to your ICUBA benefits, you’ll be required to verify your dependent(s) meet the Eligible Dependent Definition. Dependents who turn 26 during the year lose eligibility for coverage and will be removed your plan on December 31st (the end of the calendar year they turn 26).

Eligible dependents include the following:

  • A legal spouse or registered domestic partner.

  • A child, stepchild, or legally adopted or foster child.

  • A child required to be covered pursuant to a Qualified Medical Child Support Order (QMCSO).

  • A child who resides with you as their court appointed legal guardian.


Domestic partner benefits are typically post-tax and may result in imputed income for tax purposes. Contact your Human Resources or Benefits Department to confirm taxability of domestic partner benefits.

Domestic partner coverage varies by school and may require:

  • Registration with your county or municipality

  • A notarized domestic partnership affidavit

  • Supporting documentation like shared lease, ID, or financial records


Adult Disabled Dependent Children🧑‍🦼

Adult disabled children may continue coverage beyond age 26 if they meet IRS and ICUBA's plan requirements. ICUBA's defines adult disabled children as:

  • A child of the plan member enrolled in the plan when they attain age 26.

  • Mainly dependent on the plan member for support.

  • Unable to provide self-support due to a mental or physical disability.

  • Claimed as the plan member's dependent for income tax purposes.**


Members with adult disabled dependents must provide a Disabled Dependent Affidavit for any dependent children over 26 who are covered under medical, dental, or vision with ICUBA. If you previously submitted an affidavit, it is not required again. ICUBA may, at its discretion, request additional documentation to support the enrollment of a disabled dependent.


*If not claimed as a dependent for income tax purposes, dependent must be eligible for Social Security income or Supplemental Security Income (SSI) as a disabled person or is working in supported employment which may offset the Social Security or SSI.


Additional Notes 📌

  • Missed premium payments, voluntarily dropping COBRA, or termination due to fraud do not qualify as QLEs.

  • Requests to change benefits cannot be honored if the request is made more than 30 days after the life event date.

  • Documentation must be provided to substantiate life event changes.

  • You may only enroll one domestic partner per 12-month period.

  • Find your plan details, request ID cards, or view claims 24/7 via the ICUBA Benefits Portal.
